Pick up forms in advance from the office if you would like. 7-8th grade, per sport $30 9-12th grade, per sport $55 Family Max $200 Make sure signed FB Athlete Handbook, signed MSHSL, updated Sports Physical, up to date Impact test and fee is paid before practice begins.
